  • This card is valid for primary card holder and 5 household members as filled on form.
  • Privilege member can avail the benefit of report by email/ whatsapp.
  • Updates on new packages and schemes through email or SMS.
  • Every member will be issued a unique ID. Which will be linked to your details. It will give advantages of quick registration of patients demographs while attending branch.

  • Membership is nontransferable; however patient can add five members of his family under City X-Ray and Scan Clinic privilege membership loyalty card.
  • City X-Ray and Scan Clinic reserve the right to change discount pattern or scheme at any time.
  • City X-Ray and Scan Clinic can cancel membership or discontinue privilege membership loyalty card program at any moment of time without prior intimation.
